In consideration of my entry being accepted, I hereby (for myself, any heirs and executors) waive all rights and claims for damages which I may levee against and release and indemnify War for Wellness and all other sponsors, volunteers or any other affiliates, relating to or arising out of my participation in the 2010 War For Wellness Family Fun 5K Walk/Run even though that liability may arise out of negligence or carelessness of the entities or persons named in this waiver. If I should suffer injury or illness due to participation I authorize the officials of the event to use their discretion to administer First Aid and/or have me transported to a medical facility and I take full responsibility for this action. I attest and verify that I am physically fit and hereby grant full permission to any and all of the foregoing to use any photographs, videotapes, motion pictures, recordings or any other record of this event that may include the use of my voice or likeness for the purpose of promoting and archiving the 2010 War For Wellness Family Fun 5K Walk/Run. I have read the above release and understand that I am entering this event at my own risk.
